Starting March 25th 2013
Monday-Friday from 8am~5pm, Saturdays 9am~2pm,
Sundays 12:30pm~4pm.

We are pleased to announce our practice has been accredited as a Level 3 Patient Centered Medical Home with the National Comittee for Quality Assurance as well as Stage 1 Meaningful Use of Electronic Health Records. Here at Charter Oak we strive to improve the quality of our patients' wellness care.

At Charter Oak, we offer walk-in care as well as
a limited number of scheduled appointments each week day.

Feel free to call our office regarding the services
we provide or to schedule an appointment.

We are currently accepting new patients for primary care services.
